Original Contribution to the Art and Science of Orthodontics
1. Took as occasional clinical accident or curiosity and developed the rapid palatal expansion technique. Introduced same in the 1961 publication. The RPE technique has become the most significant and most utilized orthopedic technique in orthodontic history.
2. Was the first to use and report on the application of heavy cervical gear force to orthopedically correct the skeletal Class II. (1970 publication)
3. Introduced the orthopedic crib to unitize the maxilla to facilitate the use of orthopedic force in the mixed and transitional dentition. (1970 publication)
4. First to use the expanded Kloehn cervical face bow appliance for slow palatal expansion.
5. First to call attention to fact of apical base expansion being the most important objective attainable in orthopedic orthodontic treatment.
6. First to distinguish the dramatic and important differences between maxillary apical base expansion (very stable) via slow or rapid palatal expansion and mere dental arch expansion. (very unstable)
7. First, to introduce, describe and utilize the concept of enlarging the apical base in the A-P by influencing buccal segments posteriorly into maxillary and mandibular growth cites via the cervical gear appliance to one arch and elastics from the opposite arch worn to the cervical gear e.g. the tandem mechanics.
8. Introduced the concept of protracting the maxilla after disrupting it's hafting articulations with RPE. The appliance, introduced in the 1970 publication, to accomplish this objective is the protraction chin cup.
9. Extended the use if the protraction chin cup to protract recessive dentures or to prevent them from becoming recessive as in closing large spaces due to missing teeth.
10. First to use the vertical pull chin cup to control vertical skeletal anomalies by; 1. preventing buccal tooth eruption, 2. introducing buccal teeth, 3. reducing buccal alveolar bone height, 4. moving the maxilla upward and forward, 5. changing mandibular morphology. All these happenings of course result in positive mandibular rotation and decrease in anterior facial height.
11. Was the first to demonstrate a method by which skeletal vertical could be increased in the hypodivergent facial type by torquing and protracting the maxilla following it's mobilization with RPE.
12. The 1970 publication was the first in orthodontic history to demonstrate dramatic and stable skeletal changes in all three dimensions; 1. increase of apical base in the transverse deficiency case 2. increase and decrease of skeletal vertical, 3. retraction and protraction of the maxilla.
13. The 1980 publication demonstrates a stability of treatment never before recorded in orthodontic history. The key to this stability was maxillary apical base enlargement.
14. One of the first if not the first to point out and to demonstrate mechanics to intrude or prevent incisor eruption to correct deep anterior overbite. Certainly the first to prove that such correction is remarkably stable vs. the instability so often recorded in cases treated by elongation of buckle teeth.
15. In the German publication 1992 demonstrated a splint that is highly effective in eliminating clenching and bruxing (in my opinion kenetic clenching) by utilizing the concept that the proprioceptive-reflexive system which protects our lower incisors from occusal trauma can be utilized by transferring the entire clenching pressure to one lower incisor tooth, thus causing the protective mechanism to relax the muscles of mastication.
16. Introduced the six keys to orthodontic-orthopedic stability in 1980.
I. Make the treatment fit to the patient's needs and beware of the pitfall of making a patient fit a treatment based on factors other than biology.
II. Employ a system of diagnosis which identifies all skeletal and dental anomalies.
III. Utilize growth as much as possible, where applicable, in treatment planning and in the non-grower simulate a growth oriented treatment plan.
IV. The treatment plan must incorporate a specific remedial measure for every identified anomaly both skeletal and dental.
V. In virtually every case plan to extend to your patients the most important orthodontic-orthopedic benefit available to them namely to promote apical base development to full potential.
VI. Do not squander the efforts of treatment with anything less than a well planned and long retention.
